They also care for their younger brother, aged 3. Their mother passed away one year ago from a motor vehicle accident. Before this, her parents had a very unhealthy relationship, and it was often violent; physical, and verbal. Her parents used drugs regularly, including alcohol and the open use of cannabis and speed. Due to this, her older brother moved away from the family home to live with his boyfriend when he was 16. He has since separated and lives independently with a secure position in a Childcare centre. Following the death of their mother, their father was unable to care for Rebecca and their younger sibling. He had increased his intake of illicit substances and was using them in front of the children. He would often have friends over who also took illicit substances and were often passed out in the lounge area. Rebecca reached out to her brother for help with their little brother as she couldn’t attend TAFE and look after him. Since then, they have joined their older brother, who has assumed their care. Rebecca had described that she experiences anxiety and depression-related systems, often breaks down in tears, gets frustrated easily, and has missed considerable amounts of TAFE, where she is hoping to achieve her high school certificate after leaving high school when her mother passed away. Since they are in the care of their older brother, their younger brother attends daycare daily, and his care is shared between her older brother and Rebecca. Rebecca can now participate in schooling at TAFE but struggles to return to a structured study routine and misses her old high school. She has an above-average IQ. However, she has very poor academic results. She was also reprimanded for vaping in the toilets at TAFE. Her interests are upcoming fashion and house music. However, this has recently changed to more slow, sad music (sentimental ballads) and social media, allowing her to connect to her old high school friend group. Rebecca has recently had an ongoing conflict with her Brother as her brother is frustrated by her messy bedroom, late nights, vaping and lack of engagement. Rebecca fears her brother will make her move back home to be with her father. Her brother contacted TAFE for support, and they recommended that she seek the services of a General Practitioner, who referred Rebecca to the Child and Youth Mental Health Team as she has been assessed as having the following concerns: – Frequent self-harm by superficial cuts to her arms, and lighter burns to her legs – Persistent low mood, lack of energy and motivation – Poor sleep hygiene: preferring to sleep during the day, she spends a considerable amount of time each night scrolling through social media posts – Appetite is intact, although she is underweight for her height and build (BMI 16) – Refusal to be put on any medications, as she “wouldn’t remember anyway” – She experiences uncontrollable and explosive outbursts of anger, which she said got her suspended from her previous school – She is expressing a desire to try illicit substances to escape the frustration of her life circumstances – She also expressed that she has challenges trusting people – She also shared that she is attracted to another female student, and doesn’t know how she feels about these first-time feelings and doesn’t feel that she can talk to anyone about it.
