Create a narrative of student nurse on medication errors that focuses on a case study that has been developed from the clinical experience. The case study will explore and discuss a specified clinical risk that caused a clinical incident or near miss. The case study will include a critical examination of the factors that impacted patient safety, such as interprofessional teamwork, skill mix, adherence to safe practice and relevant clinical governance principles. The clinical risk should be linked to the relevant NSQHS Standards. Students will then reflect on their own preparedness for clinical practice. Possible examples of areas of identified clinical risk leading to an incident could include (but is not limited to) medication errors, falls, pressure injuries, clinical deterioration, infection control issues, patient identification errors or communication breakdown. Content in this narrative: An overview of the chosen case study. Overview should include a description of what happened during the incident or near miss, the location (where), who was involved, the student’s role, and the outcome. A brief overview of the clinical risk identified relevant to this case study, including a classification of the risk/incident in the form of a HARM score or risk matrix. Discussion surrounding the contributing factors in this case study that may have led to the clinical incident or near miss through a root cause analysis (RCA). A fishbone or swiss cheese model must be used. Include in your discussion interprofessional teamwork, skill mix, adherence to safe practice and relevant clinical governance principles that impacted patient safety in this scenario; A reflection (using a reflective cycle such as Gibbs’ or Borton’s) on your perspective of the clinical incident and evaluate your own readiness to provide safe care (i.e. career-long learning, accountability, leadership readiness, working as part of a team).
