He was recently admitted to the emergency department due to chest pain. He was WDWN, pale, and anxious upon arrival at the ED. His review of systems is unremarkable. According to his wife, the chest pain started 3 hours prior to consult. The patient was washing his car when he felt chest pain which he characterized as squeezing like sensation and was not relieved by rest. Upon checking his vitals, blood pressure was 150/100 mm Hg, heart rate was 80 beats per minute, temperature was 36.5°C. A laboratory request for his complete blood chemistry was requested by his cardiologist which revealed his total cholesterol was 220 mg/dL; HDL 85 mg/dL and LDL 135 mg/dL, troponin | 8.6 ng/mL, and serum creatinine 0.3 mg/dL, while all other values are non-remarkable. His ECG was also taken and is shows ST segment elevation, as seen in Fig. 1 The patient has been previously diagnosed with hypertension (2003), diabetes mellitus, and dyslipidemia (2004). He has also undergone PCI 3 years ago. His maintenance medications includes Amlodipine 10 mg PO OD AM (taken this morning) ; Atorvastatin 20 mg PO OD QHS (taken last night) , Aspirin 175 mg PO OD (taken this morning). He has no known allergies. Patient does not have HF. He denies smoking, and use of illicit drug and only drinks alcohol occasionally. He has a height 5’2″ and a weight of 180
