PATIENT: HAYMAN, ALANA ACCOUNT/EHR #: HAYMAL001 Attending Physician: Olaf Andersson, MD Chief Complaint: Uncontrolled palpitations with dyspnea HISTORY OF PRESENT ILLNESS: The patient is a pleasant and elderly 78-year-old Caucasian female with atherosclerotic peripheral vascular disease. She is admitted to the hospital today with new onset uncontrolled atrial fibrillation. She presents with symptomatic palpitations, shortness of breath, and generalized weakness with exertional dizziness. She denies exertional angina pectoris, although her exertional dyspnea is now quite limiting with her palpitations. No dependent edema. She reports a several-month history of worsening left-leg claudication. A previous arterial ultrasound has documented a stenosis in her proximal left-leg arterial system. PAST MEDICAL HISTORY: The patient is allergic to PENICILLIN and PREVACID. Environmental allergies to feathers and trees. Gastric intolerance to Mevacor. Historic hypercholesterolemia and peptic ulcer disease. Recorded illnesses are hiatal hernia, gastroesophageal reflux disorder, osteopenia with lumbar compression fracture, and controlled but recurrent supraventricular tachycardia (SVT). Status posthysterectomy and bilateral oophorectomy, and right segmental mastectomy. FAMILY HISTORY: Both the patient’s mother and brother are deceased secondary to colon carcinoma. SOCIAL HISTORY: Patient is a widow, currently living alone. She is a retired biostatistician. No recent tobacco usage.
