Ms. Rose is a 70 y/o female with a diagnosis of COPD and CHF. Ms. Rose is on 2L O2. She is walking with independently with a rolling walker. She has progressed with self-initiation of activity pacing and planning strategies. Ms. Rose is able to perform LB dressing using a dressing stick, reacher, and sock aid independently. She requires set up for shower and verbal cues for rest breaks. During a kitchen activity last week, Ms. Rose forgot the bread in the oven, and it burned. She required verbal cues to check the bread. Her vision is WNL with glasses. She wears dentures and is able to feed herself without assistance. Pt has met the following goals; Independence for UB and LB dressing Independence with rolling walker for toileting and toilet t/f. Independence for grooming standing at the sink. She has not met the following goal: Independence for meal prep as she still requires supervision. During the course of her stay you provided training on activity pacing and planning, ther ex for bilateral UE and lower extremity, standing tolerance activities, ADL training, and training with the use of adaptive equipment.
