REASON FOR VISIT: Patient is here for preoperative clearance. Dr. Glendale is the surgeon to repair an aneurysm behind his left knee. He denies any cardiac discomfort or SOB at present. HISTORY OF PRESENT ILLNESS: This 61-year-old male is scheduled for popliteal aneurysm surgery per Dr. Glendale. The patient has a history of thoracoabdominal aneurysm operated on by Dr. Glendale approximately 1 year ago. Catheterization at that time revealed the right coronary artery to be a dominant vessel diffuse plaquing with two areas of eccentric stenosis of approximately 60-70% in the midportion. The circumflex was a small and nondominant vessel. Ejection fraction was approximately 6%. Medical management was recommended. At the present time, there are no symptoms of angina, heart failure, or arrhythmias. The patient remains compliant with his diet ICD 10 CM & PCS CODE
