In the following 5 scenarios, you are asked to: Step 1 – Identify the type of question being asked in the HIM question query. There are 4 possible choices for query: POA – The query seeks to resolve the question of POA. Secondary – The query seeks to clarify a secondary diagnosis. Principal – The query seeks to clarify the principal diagnosis. Not Necessary – There was no purpose for the query. It was not necessary and should not have been sent to provider. Step 2 – Assess if the healthcare provider has responded to the query in a complete, allowable, and helpful fashion. There are 4 possible choices for healthcare provider response: Unresolved -The provider response does not answer the query – the question posed is unresolved. Complete – The provider response is complete, allowable, and helpful. Not Allowed – The provider response is present but contains content that is not allowed or content that is not appropriate. Second Query – The provider of the care did not respond. There is no provider response. A second query is necessary. Step 3 – Establish your two choices made in the above two steps. There are 16 possible combinations from Step 1 and Step 2. Step 4 – Mark the letter that matches your answer for the scenario by reviewing the 16 possible combinations. ALL 5 scenario answers will have ONE letter (A-P) selection for the answer and is worth 4 points.
