These factors are all critical determinants of health, shaping disease prevalence, access to  care, and health equity. DNP-prepared nurses can address the root causes of poor health outcomes, rather than only treating symptoms, by embracing SDoH. Patients who have uncontrolled diabetes may not be able to afford their medication or be able to have access to healthy food rather than tagging them as non-compliant. Understanding these factors allows for more comprehensive, patient-centered interventions that promote long-term well-being. Selected SDoH Domain: Economic Stability Among the five SDoH domains, economic stability is crucial to me, as financial hardship  is a significant barrier to healthcare access and adherence. Lower-income individuals are especially at risk for food insecurity, unemployment, and limited access to healthcare services, all of which contribute to chronic diseases and early mortality (HHS, 2023). I have patients who  skip doses of lifesaving medications or preventive care because they are too expensive. For example, a patient with  hypertension may refrain from filling prescriptions because they do not have enough money, leading to emergency department visits for conditions that could be prevented. DNP-prepared nurses can significantly impact addressing economic barriers to accessing and affording healthcare through financial assistance  programs, partnerships with community organizations, and advocacy for policy changes. Shifting from Patient-Level to Broader Organizational and Community Impact While addressing SDoH at the patient level is crucial, systemic change necessitates focusing on  the organizational, community, and policy levels. I will make  the following changes to my practice: Organizational level: I will work with hospital administrators to implement financial hardship screening tools and establish referral systems for patients who need financial or social support. Research by Gottlieb et al. (2023)  highlights the importance of integrating social determinants of health (SDoH) assessments into electronic health records (EHRs) to identify at-risk populations and customize interventions as appropriate systematically. Community level: Another approach at the community level would be to collaborate with local organizations to enhance the economic stability of nearby residents through employment programs, financial literacy workshops, and food  assistance initiatives. A study  by Marmot and Allen (2023) shows that community-based interventions can effectively address economic disparities and improve population health. Field of Nursing: At the system level, I will work towards policy changes that reduce  the economic barriers to quality care. Billionaires may use their voice  in meaningful ways through their foundations. Research by Williams et al. (2023) emphasizes the impact of nurse leadership on health policy to address social determinants. Conclusion Understanding the role of social determinants of health (SDoH) fosters an integrative perspective that  ultimately leads to a systems approach. Therefore, DNP-prepared nurses should strive to convey this information in clinical settings to promote systemic change and reduce health disparities. As a health systems professional, I can contribute to creating a more equitable health system through economic stability, health policy advocacy,  and organizational and community health interventions. Addressing SDoH will remain a key focus to ensure improved health outcomes and that patients receive the care and support they need to thrive.
