A CT scan of a 68-year-old female shows a large ventral incisional hernia in the epigastric midline just superior to the umbilicus. The skin of the abdomen is prepped and the physician makes an incision over the hernia. Attention is turned to the midline hernia and an ellipse of skin is excised over the hernia, deepened down through subcutaneous tissue. There is a very large fatty sac and the hernia has very poor quality tissue on all sides of the defect. The 6 cm defect area of the anterior abdominal wall and rectus sheath are reinforced.
