All of the models discussed in this unit have merit within the emergency department setting. In their own way, each of the models provides a way to solve the unique problems of an emergency department. I would instruct my team to use the rational model to implement evidence-based medicine practices for problem-solving. At the same time, I would extoll the virtues of the bounded rationality model and operating within the limitations of one’s scope of practice. We would touch on the intuitive model and how sometimes situations will present themselves that will not give you the time to run through a protocol or reference the latest evidence-based medicine. Over time you will build the clinical gestalt to recognize situations and act based on what you know is right. Lastly, sometimes problems that present to the emergency department are beyond explanation. They are a presentation or problem that no one in the department has ever seen before and no one has the basis for a reference-based answer. That is where the creative model comes into play and though not the norm, our staff will need to know how to address radical or weird problems in a timely manner by using the creative process. As far as discussing things with my supervisor goes, I would likely use a scenario-based example. It may be something that happened to the supervisor or that the supervisor was directly involved in, but it doesn’t need to be.
