Case Study NURS1024 – Mrs Irene Thompson As a nursing student you meet Mrs Irene Thompson because she has been admitted to an acute medical ward with dehydration and heat exhaustion from the recent heatwave and you have been asked to ‘special’ her because she has some confusion. After reading her medical history and spending some time speaking with Mrs Thompson who has become lucid you determine the following information. Mrs Thompson is an 80-year-old woman living alone in a modest independent living flat in a western suburb of Adelaide, South Australia. She was born in England and immigrated with her parents to Australia as a child. She is a widow and has no surviving siblings. Her children live in different states, but she rarely sees them due to the distance and their busy lives. Mrs Thompson does not drive and she does not wish to travel. She has a past medical history of breast cancer and currently has hypertension, osteoarthritis, presbycusis, a macular hole, asthma, obesity, anxiety, and depression. She disclosed that she is sometimes incontinent of urine and faeces. Mrs Thompson worked as a part-time cleaner throughout her life, but this was physically demanding work and her declining health forced her to take early retirement. Mrs Thompson lives on the aged pension and has no savings, which limits her financial resources. She stated that she struggled to afford the rent, food, medications, and heating during the winter months. Her low income and osteoarthritis affected her accessing healthcare services and she didn’t want to bother the general practitioner. A council worker did her shopping, but this was the only help she received. Mrs Thompson’s rented flat is small and is not insulated. During extreme weather, she is challenged in maintaining a comfortable living environment. She does not have any air conditioning, only a fan. The risk of falls is increased due to uneven flooring, steps for access, and inadequate lighting. She stated she was able to mobilise around her flat but rarely went outside for fear of getting ‘caught short’. She spent most of the time sitting in a chair looking out of the window with a view of the neighbour’s fence. Activities such as watching TV or reading were limited because of her eyes being tired and her loss of hearing. Mrs Thompson stated that her memory was not very good. Mrs Thompson had limited social interaction and all her friends had died, and she stated this contributed to her feelings of loneliness and depression. She rarely participated in community activities or did not attend any social gatherings. A neighbour sometimes checked in on her, but she stated she did not like her. She talked to her two children on the telephone occasionally. Although Mrs Thompson believed her children cared about her, their visits and contact were infrequent (unless they wanted some money). She felt her grandchildren were strangers. Mrs Thompson grew up during a time when the educational opportunities for women were limited and she left school when she was 13 years old. Her lack of formal education has affected her understanding of her health. Early childhood experiences for Mrs Thompson included growing up in a family where her father was often unemployed and drank heavily. Her father was a soldier in the second world war and was sometimes violent towards her mother, herself, or her siblings. Assessment 3: Case Study Report: For this assessment you are to make a report on four aspects of the case study of Mrs Irene Thompson. There are four sections to this assessment as follow: An introduction and conclusion are not required. A reference list is required. The following headings are to be used Psychological Theory, Sociological Perspectives, Fundamentals of Care Framework, and Self-Reflection. 1. Fundamentals of Care Framework Using the chosen psychological theory and the sociological issues identified and applied to the case study, relate these to the Fundamentals of Care Framework domains including the relationship, integration of care, and context of care domains (Feo et al, 2021). It is expected you will describe the domain and the elements within the domain and how these apply to the case study of Mrs Thompson. 4. Self-Reflection Identify at least three (3) of your core attitudes, beliefs, or values in relation to nursing an older person such as Mrs Thompson (you can use the first person for this self-reflection part). Describe these attitudes, values, or beliefs and why they are important for nursing care with due referencing. You may like to refer to the Module 12 reading by Stephens & Brighton (2021) in advance of the pre-tutorial work. Finally, analyse how your values and beliefs may have shifted over the course of the topic. Ensure all arguments and opinions are supported by relevant literature.
