Anusha Singh, a 15-year-old girl of Singaporean heritage, was born in Australia and is currently a Year 10 student at a local school. Active in her school’s netball program, Anusha is also managing Type 1 Diabetes Mellitus (T1DM), a condition she was diagnosed with at the age of five. She uses an insulin pump, along with a pre-mix insulin regimen, to control her blood glucose levels. Recently, Anusha presented to the emergency department (ED) of a regional hospital with hyperglycaemia. This episode reflects the challenges she faces in maintaining stable glucose levels due to her active participation in sports. Cultural sensitivity in her care is important to understand the familial and dietary practices that may influence her diabetes management strategies, ensuring that the approach aligns with her family’s background and traditions Anusha Singh experiences significant challenges in managing her Type 1 Diabetes Mellitus (T1DM). Limited access to specialised healthcare services such as endocrinologists and diabetes educators makes it difficult for Anusha to receive ongoing support and education, hindering regular monitoring and delaying the management of complications. The regional location also impacts the availability and affordability of essential diabetes supplies, including insulin and glucose monitoring devices. On top of these logistical hurdles, Anusha struggles with the complexities of adolescence, particularly the social and psychological challenges stemming from feelings of isolation. Current situation Anusha presented to her local hospital with her parents, who were deeply concerned after she exhibited troubling symptoms following a netball game. Initially, she experienced frequent urination and excessive thirst, which progressed over the next two hours to fatigue, body aches, and nausea. Her mother observed that Anusha had difficulty concentrating and following simple instructions. She also complained of abdominal pain and could not keep fluids down, resulting in persistent vomiting, which occurred three times before reaching the emergency department. Additionally, her mother noticed a fruity odour on Anusha’s breath and detected ketones upon completing a finger prick blood test.
