Enhanced quality of services with more successful treatment outcomes Higher overall client satisfaction. Short Brief Intervention and Referral to Treatment (SBIRT) to escalate higher need clients to appropriate levels of care. Immediate identification and referral of clients in need of higher levels of care. The practice-level logic model outline does not fully represent every problem within my agency, but it is a snapshot of the bigger picture problems impacting clients and employees the most (n.d.). Staffing shortages are leading to limited access to care due to delays in responding to client inquiries and sitting on long waiting lists (Dudley, 2020). When clients are finally assigned to overworked clinicians, they are provided less than adequate support and attention toward their specific needs. Evidence of the need can be discovered through client satisfaction surveys, online reviews and anonymous employee satisfaction surveys. For positive changes in worker retention to occur, the agency must invest in the well-being of their employees. Providing benefits, support, training and growth opportunities will mitigate staffing shortages, high turnaround and ultimately dissatisfied clientele. Additional solutions involve flexible recruitment and retention programs, flexible staffing models, increased training and development, a wellness system designed to respond to staff needs and flexible action plans to ensure safe patient care (2022). The organizational culture has a profound impact on employee retention (Long et. al., 2023). Improving the culture will inevitably lead to a more motivated team, an up-tick of qualified employee candidates, greater client satisfaction, and an overall better reputation for clients, stakeholders and the community. Enhanced availability of suitable resources and assistance Improved general health and a smooth transition back into society A practitioner-level approach for patients undergoing residential treatment is outlined in the above logical model, which emphasizes the importance of customized treatment plans and in-depth needs assessments. According to Dudley (2020), a well-conducted needs assessment has the potential to significantly boost the effectiveness of social work initiatives. The needs of each client can be ascertained through the use of surveys and CRAFT, which leads to the development of customized treatment plans. Another key element of the plan is the creation of a guidance center for patients with complex care needs. This facility wants to provide more guidance and support to its clients while making sure they have the resources they need to succeed in their rehabilitation. “One solution is to use decision aids and supports to help patients make medical decisions. These tools can ensure patients receive information in a standardized format that includes all critical content, presents information in a culturally appropriate manner, and uses language and images to ensure understanding across a range of health literacy needs.” (Krist, 2017) By addressing these unmet needs, the proposed approach aims to improve the short- and long-term outcomes for patients undergoing home therapy. Enhanced program satisfaction and client engagement are two of the immediate outcomes that improve therapy compliance. In the long run, these therapies might hasten the healing process and facilitate a patient’s successful reintegration into society. Numerous resources, including the Community Toolbox, the Quality Implementation Framework, and evidence-based healthcare decision-making manuals, have an impact on the interventions described in this logic model. There are many advantages to involving patients in healthcare decision-making. Patients who are involved in the decision-making process express greater satisfaction with their care; they also know more about conditions, tests, and treatments; they have more reasonable expectations about benefits and risks; they are more likely to follow screening, diagnostic, or treatment plans; they experience less anxiety and decisional conflict; they are less likely to undergo unnecessary tests or procedures; and, in some cases, their health outcomes are even better. (Krist, 2017). By following the counsel and recommendations from these sources, the proposed intervention aims to fill in the gaps and fulfill the needs identified in the residential treatment setting, ultimately improving the overall wellness of clients who are in receiving services and what to be present for their decision making.
