Jack – Case study 1, Cancer Jack is a 13-year-old boy, the only child of John and Regina, a professional couple who encountered difficulties when trying to have children in their late 30’s. Jack was diagnosed with Acute Lymphoblastic Leukaemia at 10 years of age. Since completing his treatment there has been no recurrence of his cancer, however he suffers from ongoing adverse effects of treatment. His primary problem, when he presents to your clinic, is ongoing fatigue, “brain fog”, and difficulty with concentration. Prior to his treatment, Jack was a capable and engaged student at school and he is frustrated by his inability to perform at his best, academically. After school each day he is exhausted and only wants to sleep, which limits his capacity to complete the large volumes of homework required by teachers at his expensive private school. Whereas he had previously been a committed and capable member of the school’s water polo team, he is now unable to dredge up the energy required to participate in organised sports at school or on weekends. While Jack is frustrated by the limitations of his fatigue, he finds that his parents’ response has been unhelpful for his management of his fatigue. John and Regina tend to be over-protective of Jack and the possibility of his death due to his cancer diagnosis remains a great source of distress for them, even though the likelihood of relapse and of a poor outcome are now remote. Jack thinks that there must be some way of addressing his fatigue and that he might be capable of higher levels of activity than his parents are willing to allow. It is clear from your conversations with him that Jack would very much like to find a way to manage his fatigue that will enable him to perform both academically and in sporting competition at much higher levels than he is currently capable. 1.
