Amy has come to her appointment with the community health nurse. You are a student on placement with the community child health nurse. Introducing the family This is family lives Burnside, near Nambour. They live in a mortgaged house. Amy (16 years). She is in year 10 at Burnside State High School. She is an only child and has been diagnosed with epilepsy at a young age. Amy lives with her mother Olivia and her step father Ben. She achieves good grades and hopes to go to university to study nursing when she f inishes school. Amy plays the violin in the school band and has band practice each Tuesday afternoon. Olivia (mother, 45 years), is a part-time medical receptionist, working 3 days a week. She divorced her husband Brad in 2009 due to an extra-marital affair and has limited contact with Brad. She married Ben in 2011. She enjoys going to the gym after work to help her destress. She has coffee every weekend with her mother, Evelyn who lives locally. She is concerned lately that Amy is becoming withdrawn and is anxious that Amy has experienced a seizure as her epilepsy is normally well controlled. Ben (step-father, 46 years), is the youngest child of his family. He has an older brother who lives in South Australia which he contacts every month. Ben works full time as a physiotherapist at the Sunshine Coast University Hospital. Ben enjoys playing social football with work friends each week. Extended family Brad (Amy’s biological father, 48 years). He is an only child. He has limited contact with Amy and Olivia after separating in 2009. Evelyn (Amy’s Grandmother – Olivia’s mother, 75 years, retired). Lives in an over 50s housing with her husband Henry in Nambour. Has coffee with Olivia every weekend and visits frequently with Henry. Henry (Amy’s Grandfather – Olivia’s father, 76 years, retired). Married and lives with Evelyn. Visits the family with Evelyn once a month. Liam (Ben’s older brother, 48 years). Liam works as a site manager for a water processing plant in South Australia. Liam has a new partner, Jackie. Liam and Jackie visit twice a year. Waylen (Ben’s father, 68 years). Recently retired as a police officer. Married to Camila for 49 years. Lives in South Australia and visits for Christmas. Camila (Ben’s mother, 67 years). Married and lives with Waylen. Gil (Amy’s biological grandmother – Brad’s mother, ? age). No contact. Married to Tom. Tom (Amy’s biological Grandfather – Brad’s father, ? age). No contact. Married to Gil. Social supports School – Amy attends. She is in grade 10 at Burnside State School. The school nurse is aware of Amy’s epilepsy and Amy has visited the school nurse for epileptic episodes before. Band – Amy enjoys playing the violin and has recently made new friends, like Adam, from her band practice. Church – Amy, Olivia and Ben attend church service every Sunday at Burnside Family Church. The family has been attending this church service for 9 years. Grace – (Amy’s best friend, 16 years). Attends Burnside State High School and has been firm friends with Amy since junior school. Amy and Grace often study for exams and complete homework together. They share an aspiration to become a nurse. Adam – (Amy’s friend, 16 years). Attends Burnside State High School and has become a recent friend through senior school band practice. Presenting health issue: Epilepsy Amy has been diagnosed with epilepsy at an early age. This is usually well controlled with medication and routine check-ups with her General Practitioner (GP) Dr Franklin. Amy has also visited the school nurse before and the nurse is aware of Amy’s condition. Olivia made an appointment to see the community health nurse as Amy has experienced a seizure at school which is uncommon for her as her epilepsy is normally well controlled. Amy states that she is taking her medication but sometimes forgets when she is busy. She describes she has a lot of stress in preparing for exams and in preparing for the Burnside State High School Band entry to the Sunshine Coast Eisteddfod. Amy also discloses that she is teased at school for her epilepsy and that she is frustrated that her mother will not teach her to drive like the rest of the kids at school. She feels like her mother is too protective of her and wants to be ‘normal.’ Both Olivia and Ben are concerned that Amy has become withdrawn. Oliva is notified of any health concerns for Amy and has become very concerned since Amy had experienced a seizure at school. Evelyn and Henry have offered to stay with the family help Olivia get some sleep. They have also offered to have Amy stay for a week but Amy has declined.
