S/P New Admit N#1, Ms Martello is a 44-year-old Caucasian female admitted from Eastern Long Island Hospital to the ward on 8/5/24 in the AM. She is alert and oriented x 3 and able to verbalize her needs to staff. No acute distress noted. No c/o voiced. Patient admitting diagnosis is Major Depressive Disorder, Axciety disorder, Post Traumatic disorder, boarderline personality disorder, and alcohol abuse. Ms Martello ambulates with a steady gait. She presents with calm and interacts appropriately with staff and peers. She was compliant with medications and lab work. Patient denies A/V/T hallucinations and suicidal/homicidal ideation. She did not voice any plan/ intent to self harm. Pt encouraged to verbalize her feelings. She had a good night sleep. No duisturbances reported. Her last BM was 8/05/24. Ms Martello is on 1:1 x24hs to prevent SIB. No incident reported. She only consumed the cereal on her breakfast tray despite education and encouragement. Monitoring ongoing. Safety and therapeutic milieu provided.
