It is an abnormally cold December day, and you are at the holiday party on your unit. Today is ugly sweater day and everyone is glad to be wearing them due to the weather (but they forgot to tell them to turn down the heat). The staff have all brought their favorite foods. This includes main dishes: mac & cheese, enchiladas- green and red, Kung Poa Chicken, goulash, lasagna, Greek splinantero, and a whole smorgasbord of international favors. Then there are the desserts: Chocolate walnut bunt cake, Hotteok (Korean Sweet Pancakes), Cherokee Blueberry Honey Cake, fruit strudel drizzled with icing and walnuts, and some baklava (to name a few). To follow there is eggnog topped with a sprinkle of freshly grated nutmeg, wassale, and a homemade nut milk sprinkled with Ghirardelli Chocolate powder and a dab of fresh vanilla flavored whipped cream (a personal favorite). Nobody ate breakfast anticipating today’s feast and the party has started. The plates are being piled high. Some for want and some for curiosity. You notice that Mary, out-going nurse she is, who is known for bring food and feeding the unit, is being very selective today. Several staff have been giving her a bad time for making them fat while she was watching her weight. After about 30 minutes you see Mary in the corner. She looks flushed and slightly frantic in her eyes (she is always calm in every emergency). You approach her and she is holding her chest and having some difficulty breathing. She looks in your eyes and you see fear. She can’t seem to get words out. Scenario: 38 y/o female, flushed, holding chest, having SOB, wheezing, and difficulty talking. Onset 1-2 minutes. Mary is: 1- Is having a reaction due to the closeness of the staff, hot sweaters, and warm room. She is suffering from heat exhaustion. 2- Mary is having an anxiety attach. 3- There is not enough data to identify the problem, but something is wrong. 1- Incorrect- Heat stroke- Although heat stroke does cause red flushed skins, shallow breathing, and behavior changes. It is also is related with rapid heart rates, nausea and vomiting, dry skins and dizziness normal following long periods of exertion without fluids. 2- Incorrect- Anxiety attacks- Nauseated, sweaty, shaking, palpitations, SOB, and agitation. 3- Correct- It is to early to know…but what are you going to do for her?
