Jonathan is an 18 year old male who has a diagnosis of Autism Spectrum Disorder. He has limited communication skills and can be resistant to change. He lives with his mum, who is his carer and has been assisting Jonathan with his ADL’s his whole life. His mum is seeking assistance with increasing Jonathan’s independence so that he may move out of home and into assisted accommodation in the future. The client has recently participated in an assessment of his independence in activities of daily living with an external service provider. The assessment was performed using the Katz Index. Using this index a score of 6 indicates full function, a score of 4 indicates a moderate impairment and a score of 2 or less indicates a severe impairment. You are to prepare for your upcoming meeting with the Occupational Therapist. List below at least 3 questions that you can ask the Occupational Therapist to clarify Jonathan’s current abilities during dressing and feeding. Before you call the client and his carer script questions that you can ask to determine the following information. • Jonathan and his carer’s availability for a weekly session for the next 6 weeks • If there are any cultural and spiritual issues that might affect Jonathan’s ability to function independently • If there are any safety concerns or risks that need to be identified prior to participation in the program. 1.5 During your phone call it was identified that the client has trouble balancing during single leg stance such as when dressing himself in a standing position. How can you decrease the client’s risk of injury whilst dressing? 2.1 Further information is required before the Occupational Therapist can determine an individualised program to suit the client’s specific needs and priorities for his feeding skills program. You would like further information on: • Jonathan’s current participation in feeding • The utensils used (with or without assistance) • Jonathan’s food preferences What questions can you ask Jonathan and his carer to elicit further information on the above points? 2.2 The Occupational Therapist will now need to verify the client’s competence during feeding including: • Grasp of utensil • Wrist control • Coordination of movement How can the occupational therapist verify the client’s competence in these tasks and how can you assist with this process? 2.3 Now that the data collection has been completed, the goals of the program can be determined. This will be a collaborative process with yourself, the occupational therapist, client and carer. What considerations should you and the occupational therapist be aware of when setting goals of care for a client? List 4 considerations below 2.4 Describe the process for selecting the appropriate methods to use in the program that build upon the client’s strengths whilst supporting the development of daily living skills 2.5 Is it within your scope of practice as an allied health assistant to design the client’s skill development within his program?
