Judy is a licensed practical nurse who works the day shift in an oncology center. Patrick, a registered nurse who lives across the street from her, works the night shift on the same unit. He often works with only one night off and fills in for other nurses who want time off. His wife stays home with their five children, three of whom are under the age of 6 years. Judy has noticed that for the past few weeks, Patrick has been less social and more distant when she tries to talk to him. He seems so tired and sluggish, sometimes his speech is even slurred in the morning during shift report. There have been several times that Judy has asked him to complete charting that he leaves undone. She is concerned that maybe he is working too much and mentions the change in behavior to the unit supervisor. The unit supervisor asks Judy if she has been giving more unit doses of injectable morphine to the clients She states that the unit supply of morphine has been refilled almost daily for the past few weeks, Judy states that the majority of the clients with increasing doses are on patient-controlled analgesia (PCA) pumps and that she is administering about the same number of single injections each day. When they check the sian-out register for controlled drugs, they note that most of the injectable doses are being given during the night shift. Some of the doses are signed out for clients who have PA pumps who would not be recelving routine single doses of the drug. Judy senses that she and the unit supervisor are thinking the same thing. 1. What are the indicators for a problem in this situation? 2.
