The scenario You are a support worket, and work for indigo Community Services and Health Hub (indigo) whe specielises in supporting NDIS(National Disability Insurance Scheme) participants to access supports to participate in community activities. Yusra’s parents (Khalid and Fatima) engaged in indigo’s services recently, and the Indigo case manager developed a Support Plan to summarise Yusra’s goals under the self-managed NDIS plan. Your client, Yusra is a music enthusiast who loves playing the piano and singing. Yusra would like to participate in community music workshops that are held at indigo Community Services and Health Hub, where musicians of all abilities come together to create music and get to know each other. Khalid and Fatima are reluctant for her to join the workshop as Yusra faces physical challenges due to a car accident that occurred when she was 18. The accident resulted in a cerebral haemorrhage, causing paralysis on her right-hand side, vision and speech difficulties, and the loss of her sense of smell. Yusra uses a motorised wheelchair with her left hand to maintain mobility and independence. Task 1: Meeting with Yusra Goal one of the Support Plan indicates that Yusra would like to develop speech and language skills to feel more confident around other people. Yusra has funded supports under a self-managed NDIS plan to enable her to access community and social activities. Role Play Criteria: 1.1 Consult with the person with disability to determine their interests, needs, ability and preferences regarding engaging with a social network and requirements for their preferred manner of participation 1.2 Identify and access community participation resources, programs, agencies, transport services, aids and equipment according to the person’s preferences and needs 1.3 Provide the person with information on community participation options, networks and services that meet their needs and preferences 1.4 Recognise and accommodate the cultural and religious needs of the person with disability 1.5 Work with the person to identify and access community options that will meet needs and preferences identified in their support plan 1.6 Support the person to access opportunities to establish connections through shared interests 1.7 Seek feedback from the person with disability, family, carer, others identified by the person, colleagues, or supervisor to ensure that the support continues to meet the current and changing needs and preferences of the person 1.8 Ensure strategies for community participation and social inclusion are regularly reviewed with the person and supervisor to facilitate positive outcomes 1.9 Monitor level of the person’s engagement in community and social networks in consultation with the person and supervisor 1.10 Collaborate with the person with disability to identify solutions to overcome barriers, in consultation with supervisor 1.11 Support the person to implement strategies to address barriers to community participation according to their support plan and preferences 1.12 Consult with the person to identify areas where advocacy is required to facilitate their preferred manner of participation 1.13 Monitor the success of strategies to address barriers in consultation with the person and supervisor
