Thomas presents several manifestations of depression, including social isolation, flat affect, soft speaking voice, weight gain, poor hygiene, and sadness (Mayo Clinic, 2024). The results of the Columbia-Suicide Severity Rating Scale indicate that Thomas is at high risk for suicide. Comments, such as “I hate myself”, and “Everyone would be better without me” are warning signs that Thomas might be considering suicide. Thomas experienced traumatic life events, including the loss of his mother at a young age, and growing up without a father. Thomas reports being bullied at school and on social media, and he may see himself less worthy than his peers. Nursing Management The priority nursing intervention is to place Thomas on suicide precautions, initiate one-to-one observation, and remove items that may be used in suicide (Holman et al.,2023). Promote a therapeutic and trusting environment. Administer selective serotonin uptake inhibitors, and emphasize the significance of adherence (SSRIs). Provide medication education, and serotonin syndrome. Monitor BP, and administer antihypertensives. Encourage healthy eating habits, weight management, and physical activity. Provide information about local support groups, and Crisis Intervention. Non Pharmacological interventions include cognitive behavioral therapy, meditation, and yoga. Promote self-esteem, encourage positive coping mechanisms, and self-care. Encourage outdoor activities. Encourage Thomas to take control of his health, and to participate in the plan of care. What specific issues would you want to take into consideration to provide for this patient’s unique care needs? One of the issues that I would take in consideration is the amount of time that Thomas spends in isolation. I would encourage Thomas to spend more time with his family, and to connect with people in their community (Holman et al., 2023). In addition, I would discuss the significance of setting healthy limits on playing video games and social media. Provide consultation services for victims of bullying. Encourage Keisha to support Thomas in his weight loss by providing healthy food choices and promoting physical activity. Promote family-centered care, involve Keisha in decision making, and empower her to provide at-home monitoring and care.
