identify a SMART outcome for the patient pertaining to your peer’s discussion. Discuss three (3) interventions that can be implemented to assist the client/caregiver in meeting this outcome. Interventions need to be supported with evidence. Today we are going to talk about stroke. Stroke happens when the blood flow to your brain is interrupted by a clot that forms in one of the arteries that supplies blood to the brain (Althoff, et, al., 2023). When this happens one can lose some mobilities such as speech or not able to move one side of the body which we call paralysis. (Althoff, et, al., 2023). When symptoms of stroke are caught early treatment is given and people are able to recover and get back to some if not all of their usual life activities and some people do recover completley (Althoff, et, al., 2023). Today I will be educating you about swallowing problems after a stroke that might hinder you from feeding properly. For the most part after a stroke the way you swallow can be interrupted and it is most important that we make sure that when you eat the food is fully swallowed and no residues left in the mouth since this can go in the wrong direction which can further complicate your recovery. (Althoff, et, al., 2023). While your are here in the hospital we will include a speech and language specialist who will do a thorough testing on you to make sure that your swallowing is not affected. The SLP will recommend the right food consistency and the right liquid consistency that is suitable for you. (Althoff, et, al., 2023). You might start off with food that is pureed and soft which will be easier for you to swallow. We will continue to monitor your progress and continue to make changes as tolerated. While eating make sure you’re sitting upright this way it is easier for you to swallow. (Althoff, et, al., 2303). When drinking any form of liquid refrain from using a straw for this will prevent you from chocking on the liquids (Althoff, et, al., 2023). While eating try to avoid distractions this way you can better concentrate on your feeding. (Althoff, et, al., 2023). Next thing I will be talking to you about today is your medications. As you prepare to go home, the doctor will prescribe you some medications that will help you from getting another stroke. One of the medication regimen will include Plavix ( Althoff, et, al., 2023). This medication helps the platelets from sticking together and will stop another clot from forming in any part of your body and travelling to your brain (Althoff, et, al., 2023). The doctor might also prescribe an aspirin in low dose form which does the same thing as Plavix. (Althoff, et, al., 2023). Another medication would be one that that controls your blood pressure such as Lisinopril to help stabilize your blood pressure. (Althoff, et, al., 2023). You will also be sent home with a cholesterol reducing medication to help stop plaque forming in your arteries. (Althoff, et, al., 2023). A stool softener might also be prescribed to help prevent constipation for this can cause increased pressure in your brain. (Althoff, et, al, 2023). I know I have given you a lot of information in a short period of time, if you need further clarification on this information please do not hesitate to call me and I will be glad to go over it with you again.
