S.H. is an 11-year-old African American female who visited the clinic with her mother, complaining of persistent difficulties with attention, forgetfulness, and organization. These issues appear to have been present since the onset of formal schooling in kindergarten and have continued to the present. Often forgets assignments unless they are written down, and even then, she frequently loses the list. She has trouble sitting still in class and tends to fidget. Her ability to focus on reading is limited, and often does not remember what she has read. Loses personal items frequently, such as her geography book and a cherished bracelet. Admits to daydreaming in school, often about her mother or playing with her dog at home. She also experiences frustration and anger when she makes mistakes in her schoolwork or feels she has not heard instructions from her teachers. Relies on lists and reminders from her mother to manage assignments, though these are not always effective, impacting various aspects of her school and home life. Also has a family situation where her mother is currently separated from her father, contributing to additional emotional distress. Finds some comfort and distraction in video games and art, though these activities do not address the core issues of attention and organization. Reports of sleeping 9 to 10 hours and having difficulty eating meals as she has a hard time sitting still and does not get proper nutrition, per her primary physician. Background: no history of treatment, developmental milestones met on time, vaccinations up to date. Sleeps 9-10hrs/night, meals are difficult as she has hard time sitting for meals, she does get proper nutrition per PCP. she has a younger brother. lives with her parents in Washington, D.C. No hx of head trauma. Provide differential diagnosis by applying the diagnostic criteria by connecting the presenting s/s to the diagnostic criteria
