Students will create a clinical assessment with diagnosis and treatment plan for a fictional character with a co-occurring disorder. This paper should include information regarding screening and assessment inventories, a bio-psycho-social assessment of the client, including spirituality and multicultural issues, a complete mental status exam, a DSM diagnosis, ASAM assessment criteria used to determine level of care, and a treatment plan addressing co-occurring disorders, with attention to the role of spirituality, family, social networks, and community systems in the treatment and recovery process, including appropriate individual, group, self-help, and family intervention strategies. You should use creative liberties for information not provided in the movie. In addition, current social situation such as employment, relationship history, family relationships and if children, current living situation, employment status. Explore client’s strengths. Summarize the overall assessment of the client, thoughts about diagnosis based on symptoms, integrating information from the assessment phase, include attention related to differential diagnosis and be clear about which disorders are co-occurring. Discuss where the client is in their addictive cycle (stage of change), and their motivation for seeking treatment. Identify the appropriate level of care, and articulate which information was used to make this determination. Summarize the treatment plan, including specifically addressing which level of care will be utilized, the type and format of counseling, recommendations for support groups, and how the integration of services will be achieved for the co-occurring disorder. Specify the treatment plan including problem identification, goals, objectives, and interventions/strategies to be utilized.
