The scenario I chose is the neuro system, regarding a 38-year-old female named Naomi Reed who was admitted to the hospital and was diagnosed with an ischemic stroke. Information that must be recognized and clinically significant by the nurse includes that the patient experienced slurred speech, left-sided facial droop, Left sided weakness, the patient has a history of smoking, and is on birth control. Her blood pressure is at 185/100. Underlying causes of ischemic stroke is caused by deficient blood and oxygen supply to the brain due to a blood clot blocking or narrowing the artery that leads to the brain. As the nurse, the body system I will thoroughly assess is the Nervous system. I will focus on a neuro assessment that will include mental status, reflexes, and testing the cranial nerves’ motor and sensory functions. Priority diagnoses for this patient would be to monitor the blood pressure and continue neuro assessments. The patient is most likely feeling weak and tired, she may feel off balance due to weakness on one side of her body. Also, she may feel upset or frustrated because she cannot communicate properly because she slurs her words and facial droops affecting her appearance.
