What are the key findings for ihuman case of 69 y/o who comes in the out patient clinic with chest pain, has history of grade II systolic mummur, HTN, CAD and hyperlipdemia, Vital signs pulse 108, blood pressure both arms 154/86. During history taking patient states he been having chest pain, sometimes he gets winded and tired, reports pain heavy but it is not in one area. it is all his chest, started about 5 days ago, first started when walking around the neighborhood on his afternoon walk about 5 days ago, pain is comes on when he exercise or when he exerts himself, when he stops and rest it goes away, pain lasts for 5 minutes may be only 2 or 3 minutes, when he walks to fast it seems to make worse, when he takes a rest pain goes away. patient also states the not taking any medication for chest pain, when he rest the pain goes away, sometime he gets shortness of breath. patient’s social/leisure activity is just watching his grandchildren, walking and cooking, denies smoking, alcohol, no use of drug, no palpitations. the symptom that is most destressing is chest pain.
