Has the patient had these symptoms before? Did the symptoms begin after becoming a vegetarian? How long has she had depression symptoms? Differential Diagnosis Iron Deficiency Anemia (IDA) D50.9 – The patient’s age and diet are major risk factors of IDA. Similarly, the patient experiences several symptoms related to the disease including pale skin and conjunctiva, shortness of breath after a physical activity, and frequent fatigue (Warner & Kamran, 2022). Thus, the patient is likely suffering from IDA. Major Depressive Disorder (MDD) F32.0 – The patient could be likely depressed as she experiences extreme fatigue, memory loss, and increased sleep (Surova et al., 2021). Also, the patient’s gender and being diagnosed with the illness for several years are risk factors. Similarly, failure of treating IDA sooner exposes her more to depression (Warner & Kamran, 2022). Hypothyroidism E03.8 – The patient may have hypothyroidism due to symptoms like forgetfulness and shortness of breath (Rawat & Sharma, 2021). Therapeutic: Plan The patient must be started iron supplementation which must be taken without food to escalate absorption. The patient should limit physical activities until anemia is corrected. She should be educated on compliance with her medication and how to observe symptoms of anemia (Warner & Kamran, 2022). The patient should also be instructed on having a nutritional diet as her vegetarian diet may be lacking in enough iron. Thus, she can be advised to start taking iron-fortified milk formulas and red meat especially for her age. References Rawat, D. & Sharma, S. (2021).
