She was on Zyprexa 5mg at HS for agitation from 1/8/2025 to 1/17/2025. Per discontinue of Zyprexa, patient has been stable with no behavioral issues or anxiety. PHQ-9 done and score was 14 indicating moderate to severe depression. This is in line with the assessment done by previous provider on 1/16/25. Patient will be starting on Mirtazapine 7.5mg for depression and to help with appetite since there is >5% weight loss ( 1/8/25 was 94lbs and 2/19 weight was 88lbs). Pt verbalized she sleeps good.
