How do you respond to this situation? Reflect upon a decision you have made or a problem you have solved in the past year. Working as a med-Surg RN there are always opportunities to be involved in problem-solving and change. Not only does the RN utilize critical thinking in patient care, but in processes that impact the patient to ensure patient safety. Critical thinking is an attribute that enhances one’s skill in problem-solving and decision-making (Romeo, Elizabeth, 2010). One significant change I have been a part of is initiating our process for a new patient population we are servicing, Tikosyn and Sotalol loading patients. These patients are to be admitted for observation when starting this medication because they are at a high risk for developing a prolonged QTc, which can lead to lethal dysrhythmias. At first, our process was quite the learning curve. An important aspect of this process is obtaining an EKG two hours post medication administration to assess the QTc. The provider is then to be contacted and the RN is to email or fax the EKG to the provider for viewing and approval for the next dose. However, I found this step was always inconsistent; some were only emailing or only faxing. Others are not aware of doing either at all, placing the EKG in the patient chart for the provider to round on. Bringing this to my leaders’ attention, we implemented a system called “MUSE”. This process allows our EKG machine to transmit the EKG into the patient chart directly. This way, the provider is able to view the EKG from anywhere with EPIC access. This process has been very effective and has reduced errors and increased patient safety. Problem-solving incorporates decision-making and requires action to eliminate uncertainty (Marquis & Huston, 2021). If I were in a position such as leadership, I would have provided a better resource for staff to reference, letting all staff know where this resource was, to ensure the proper steps were followed when administering this medication. While we have an educational binder and were given a short class on this topic, there were inconsistencies with updating the provider that could have been more readily addressed. please with References.
