Interprofessional Practice Case Study Case Description: The practice is a physician run private practice in your home town. The physician has been in practice more than 25-years. He is a large man with a loud voice. He is always in a hurry, worried about money, and overbooked. His routine is to come in the office after making morning rounds at two skilled nursing facilities. He is never on time. There is another general practice physician and two nurse practitioners also working in the practice part time. Today you are in the clinic along with: Two receptionists Main physician Part time physician One medical assistant Diabetic educator New case manager. The atmosphere is tense because the patients have been waiting. Mr. Davis has already asked how much longer he will need to wait because he has to go to work. You over hear the receptionists arguing about who is going to take lunch first. The case manager then asks the main physician, “Can you help me get a login for the computer?” The physician responds with “not now I need her to set up for a procedure.” You share an office with the part time physician. As she prepares to see patients she says, “I was so mad yesterday. The MAs complained to Dr. Smith that I am too slow but they are not supportive so things take longer. I bill more, and provide better care than Dr. Smith anyways.” You then see a drug representative in the hall way talking to the physician who owns the clinic. You over hear the drug rep tell the physician that he will take the physician to Vegas if he prescribes “his” drug in five different patients. The physician agreed stating, “I would love a free trip to Vegas”. At the end of your very long busy day the MA catches you in the hall and tells you that there is a patient that needs to be seen today. She is telling you this in front of the patient. All the rooms are filled with patients so there is unspoken pressure to see the patient in the hallway. In front of you is a 82-year-old man who looks frail. The MA says Mr. Arnold is feeling weak and short of breath. You tell the MA that you will see Mr. Arnold but will only see him inside of an exam room. The MA looks angry but rooms the patient. When you go in to see Mr. Arnold he is fully clothed and the vital signs have not been done. You leave to find the pulse ox. You cannot find it. You finally find it tucked in a back room draw after a twenty minute search. It is now 630pm and you are scheduled to leave at 600pm.
