A 64-year-old female presents complaining of severe fatigue, which began 4 months ago and has gradually worsened. On examination she has moderate hepatosplenomegaly with normal examination of the heart, lungs, and skin. A complete blood count shows a leukocyte count of 150 × 109 cells/L, hemoglobin level of 9.8 g/dL, and platelet count of 424 × 109 cells/L. A peripheral blood smear shows immature leukocyte forms as well as teardrop-shaped erythrocytes and nucleated red cells (please see photo above). Cytogenetic testing is negative for the BCR-ABL1 translocation. What is the most likely diagnosis?
