Patient: JR Sex: Female DOB: 6/5/1967 Today’s HbA1c: 7.6%
H.t: 5’6″ Wt.: 215lb BMI: 34.7 Today’s Fingerstick: 189mg/dL

J.R. is a 55-year-old African American female who presents to the clinic for follow-up management of type II diabetes mellitus. She is 5’6″ in height and weighs 215 lbs. Her BMI is 34.7  Her blood sugar by fingerstick in the office today is 189 mg/dl. She has been on Metformin 1,000mg, 1 tab p.o. twice daily for 12 months and today’s HbA1C result is 7.6%. Her glycemic goal which was set in consultation with her NP is to reach and stay under a HbA1C of 7%. She has lowered her HbA1C over the past year but is not at her goal at today’s visit. Her vital signs are all within normal ranges.

Her current medications include:

Metformin 1,000 mg 1 tab p.o. bid for Type II DM (she has taken for 12 months)
Lisinopril/HCTZ 10/12.5mg daily for HTN
