Chief complaint: 17-year-old presents with fever, cough, congestion, and body aches for 1 day History of Present Illness: 17-year-old African American female with no past medical history presents with fever, chills, body aches, headache, sore throat, cough, and congestion. She states she woke up yesterday morning with a fever of 101.7 and her throat hurt. Throughout the day, she began to cough and become congested. This morning she has had a headache, fever, chills, and body aches. Her highest temperature has been 102. She has been taking Tylenol as needed for fever and headache. She states it has helped control the fever, but it has not helped much with the headache. She took her last dose was this morning at 9:00 am. She states her headache is 5/10 on the pain scale and is localized to her forehead. She typically takes Zyrtec for her seasonal allergies and states she is currently taking this medication. She states she has absolutely no energy and just wants to sleep. She has no appetite and has only been able to drink Gatorade this morning. She also states she was up all-night coughing. She denies any illnesses in her home currently. She also denies being in close contact with anyone that has been sick recently at school. She denies nausea, vomiting, or diarrhea currently. Physical Exam: On examination, vital signs: Temperature 99.9 orally, respiratory rate is 18, blood pressure 110/65, HR 110, and oxygen saturation 98% on room air. General appearance: alert, ill appearing, but in no distress. Normal respiratory effort, no accessory muscle use, and no nasal flaring. No adventitious sounds auscultated. No signs of cyanosis noted. Lips are pink, smooth, but dry. Oral mucosa is pink and moist with no intraoral lesions. Pharynx with erythema, but without tonsillar swelling or exudate. Nasal turbinates boggy. Clear rhinorrhea noted. Ear canal is clear with normal landmarks and light reflex, without discharge or erythema. Tympanic membrane translucent.
