Discussion Post: Hello Professor Snapp and class! One thing I found interesting and learned a lot about this week was the influence of historical events on many ethical research codes in practice today. For example, I am familiar with the Tuskeegee Syphilis study and its ethical concerns within healthcare. However, I was unaware of the extent of the experiments performed in concentration camps during World War II and how these actions influenced laws around consent and experimental research. The Nuremberg Code, along with the result of the Tuskeegee study and other discussed historical events, provided patients with the right to consent to research while also ensuring patients’ safety and autonomy. We also discuss the triad of evidence-based practice, which involves scientific evidence, clinical experience, and patient preference. When we talk about patient preference, as Professor Snapp mentioned, autonomy and beneficence come to mind with our patients and how the two principles may not always align. For example, in the past, I have cared for patients at clinical who refuse to turn every two hours to help prevent skin breakdown and bed sores. Ultimately, the patient has the right to participate in their care decision-making. However, one of the best things we providers can do is educate our patients on bedsore and pressure ulcer prevention and encourage patient interest in doing what will cause the least harm to them without forcing the action on them. My question branches off from the idea of patient preference and autonomy. Hypothetically, let’s say we have a patient with type 2 diabetes and a foot ulcer that may require amputation. If the patient is noncompliant with research-based measures to help preserve blood flow and function in their foot and they also wish not to have an amputation performed, what (if anything) is there that we can do for a provider to encourage patient participation in their health promotion?
