She has a history of hypertension (high blood pressure), Type 2 diabetes (not enough insulin in her blood to manage her blood sugar), coronary artery disease (CAD), osteoarthritis and has experienced a stroke (cerebral vascular accident -( either from a blood clot or a bleed into the brain) two years ago that left her with left sided hemiparesis (paralysis) and some expressive aphasia (unable to verbalize clearly what she wants to say). Her condition has been weakening over the last several months. She spends less time in her wheelchair then she previously did, requires increased assistance transferring from the bed to the wheelchair and has been less involved in the activities of the facility. Her appetite has also decreased and she is experiencing increased difficulty swallowing. She has lost 12 pounds in the last two months. When assessing her this morning, you notice that her skin is warm to touch, she is pale and diaphoretic and she is coughing up yellow sputum. Mrs. Jones is later diagnosed by the healthcare provider with pneumonia. Orders include an antibiotic, breathing treatments every 4 hours and supplemental oxygen as needed.
