NUR100 Task 3 Case study 1 Matthew Health priority: Pre-term birth. You are a student on placement working with the Community Health Nurse. Introducing the family This is a nuclear family in Buderim, living in a mortgaged house, consisting of the following: Matthew (2 weeks old) was born at 34 weeks gestation on 14 August 2024. He is under the care of General Practitioner (GP) Dr Birch at Buderim Family Health Centre and Dr Wattle at Buderim Private Paediatrics. Nicola (mother, 31), stay at home mother after leaving her part time job at the bank 4 years ago. Married Jon in 2017. Nicola usually enjoys the Buderim Library book club and volunteering at the Buderim Primary School tuckshop, though she has been unable to attend these in the past 3 months. She is stressed about raising her children alone as Jon is often working too much. Jon (father, 32), only child, but has infrequent contact with his direct or extended family due to work commitments. Jon is an engineer at a manufacturing factory in Caloundra. Jon often works 6 days a week and enjoys drinking while watching the television when he is at home. Jon has agreed to cut his drinking but finds this hard due to his work stress. Billy (brother, 6 years) Buderim Primary School. Kate (sister, 5 years) Buderim Primary School. Extended family Jenny (Nicola’ sister, 30), nurse (full time). Lives in Victoria. Married Ian in 2014. Talks to Nicola every few days on the phone and visits every few months. Ian (Jenny’s husband, 32) sports journalist (full time). Jon and Ian do not get along well. Ian is supportive of Jenny and Nicola’s close relationship. Elisa (Nicola’s mother, 55 years). Married Frank 37 years ago (1987). Lives alone and works part time as a florist since Frank past away. Frank (Nicola’s father, Died 2018). Passed away aged 52, bowel cancer. Was a school principal. Maggie (Jon’s mother, 52 years). Works as a real-estate agent in Brisbane. Visits the family only at Christmas. Robert – Jon’s father, 53 years). Works as an accountant in Brisbane. Visits the family only at Christmas. James (Jenny and Ian’s son, aged 5). Lisa (Jenny and Ian’s daughter, aged 4). Social supports Kayla – (Nicola’s old school friend, 31 years). Works part time as a bookkeeper – assists in running Jake’s electrical business. She lives in Buderim and her family often visits for barbecues each month. Her son attends the same swim club. Jake – (Kayla’s husband, 31 years.) Electrician, runs his own business. Jake and Jon get along well and go on an annual golf trip. Jon had to cancel this years trip as he was too busy with work. Jake encourages Jon to not work so hard. Zac – Kayla and Jake’s son, 6 years. Swim club – Billy, Kate and Zac all attend the same swim club 3 times a week. Nicola enjoys spending time with Kayla and has begun to socialise with a group of parents. Book club – Nicola attends a monthly book club in Buderim and enjoys discussing literature with a group of friends. She has not been able to attend recent months. Presenting health issue: Pre-term birth Matthew was born 2 weeks ago at a gestational age of 34 weeks. He was required to stay at the hospital due to his low birth weight, difficulty feeding and other concerns under the care of Dr Birch. This required Nicola to spend much of her time at the hospital with Matthew and taking Billy and Kate to school and Swim club. Jon would seldomly assist in taking Billy and Kate early to school as he felt he would be late to work. Jon is currently preparing a big presentation for a new facility and feels under a lot of pressure at work. Dr Birch is happy to discharge Matthew and Nicola to home and has provided support and equipment to continue oxygen therapy at home. Nicola is anxious about this as she feels she does not have the capacity to care for Matthew fully, feeling as if she might forget things. Although Nicola and Jon have a good relationship, Jon’s work commitments are creating disharmony in his relationship with Nicola. Nicola is concerned of Jon’s work commitments and notices he is now drinking more. Jon has agreed to cut down his drinking but finds this difficult due to his work stress and has agreed to discuss this Dr Wattle. Kayla has offered to take the kids to Swim Club for a few weeks to help Nicola. Jenny has also offered to visit and assist in taking the kids to school and preparing meals. Contemporary families are diverse. The family partnership model of care is used by nurses. It aims to support early intervention in the care of children. Health professionals must ensure that all care provision is planned around the whole family and not just the child/person. Family assessment is required to understand the way in which family’s function. To do this it is important that registered nurses understand the way in which the family functions and use strengths and coping strategies to handle stress or changes in situations. Nurses also need to clarify any problems and understand what families experience, the roles, and functions within the family, and how the families engage with each other and the community around them. In planning care, it is important to acknowledge applicable primary health and health promotion principles as well as identify any inequalities and social justice principles that may impact care delivery and/or health outcomes.
