Please proofread and rephrase this case scenario.Thanks! The patient P.P. is a 27-year Caucasian who presents to the clinic for a psychiatric evaluation. The patient stated that her mother took her in to be admitted in the hospital after four or five days without sleeping when she was a teenager. She thinks that she was hearing things at that time and that she was started me on some medications, including Zoloft, which makes her feel high and causing insomnia; risperidone and Seroquel which caused her to gain weight and Klonopin, which slows her down. The patient also verbalized that several times a year, she has experienced times when she doesn’t want to get out of bed, lacks energy or motivation to accomplish any task anything and do not just feel to use any if her creativity. She feels worthless as she is loosing her creativity and she verbalized the feeling always usually starts after four to five days of hard work on her crafts, music and painting works. Patient states, “everyone says I’m depressed, but I’m not sure. It could be that I’m just exhausted from working so hard”. The patient describes another different feeling of elation and happiness making her feel normal and that she does not need to take her medication. She will feel energized , able to perform her usual task and can go several days with little sleep but gets about her normal activites. She verbalizes that her friend’s are thinking she talks a lot or appears unkept but she feels that are being of jealous of all her accomplishments. She denies any symptoms of obsessive compulsive disorder and verbalized that she used to hear voices but not anymore. She admits getting 5-6 hours of sleep during the night with no associated bad dreams. On some days, she will feel active and sleeps for only 3 hours and sleep more on some days.
