o Group 9: A patient is postoperative day 3. The nurse notes the patient has an elevated WBC and elevated temperature. The patient asks where he/she might have an infection and how would the nurse know?

Mrs. James had her gallbladder removed 72 hours ago. Post-operatively her surgeon Dr. Wright said the surgery went well. The surgery, Laparoscopic Cholecystectomy, took about an hour, he expects her recovery should be without any complications with minimal scarring. Day 3 post operative the nurse came in to check the patient vitals. He was complaining of pain 8/10. She developed a fever of 38 degrees Celsius. When the nurse looked at the surgical site, there was excessive purulent drainage with a foul smell, redness and edema. Patient complaining of feeling chilly and is requesting extra blankets. The nurse checked the patient’s new blood results and found that her WBC is elevated. The nurse then narrows down all the causes of her symptoms to being an infection of the surgical site. The nurse then relays the information to the doctor and then orders a swab culture to determine if there is an infection. It was then confirmed that there was in fact an infection of the surgical site and the nurse then relays this information onto the patient and the patient’s daughter. The nurse communicates to the patient and family ” Because you just had a surgery I had suspicions that the location of the surgical site is where the infection might be, so we had a swab culture ordered by the doctor and it came back positive. I checked your vital signs and your Temp and WBC were elevated and those are signs of an infection, Elevated WBC means that your body is producing a lot more WBC than normal in order to fight an infection. We will give her antibiotics to fight the infection” Dr. Wright was called about the patient’s status and he came about 20 minutes later to examine the patient and wrote an order for an antibiotic for the patient. When no signs of infection is detected Mrs. James will be discharged from the hospital. The patient stated ” I do not like to look at the drainage bag and wound. I cannot do all this at home by myself” but I am willing to learn how to care for the wound and the drain. She suggested if the nurse does it a few times so she can watch and then the nurse can just be there and watch her do it and guide her a few times.

Mr James came to the ER complaining of right abdominal pain on a scale of 9/10. An ultrasound was done that showed she had gallstones. She has a cholecystectomy due to gallstones. A cholecystectomy is a surgical removal of the gallbladder. It is a laparoscopic incision using a laser. The surgery took one hour and the patient tolerated the procedure well. He was returned to PACU and will be monitored closely
