You  walk into the examining room for your next well child visit, and greet your 15-month old patient and her mother.   As soon as you speak to the mother, the baby begins to scream. The mother says, “She does this all the time lately. It’s really embarrassing!”

Yes, she is making a lot of noise, but you have nothing to be embarrassed about.  Can you tell me more about the baby’s behavior ?  I would like to establish if this is normal baby behavior or if it is a result of something occurring physically.

Why do you think the baby is acting this way?  Is she frustrated or trying to tell you something?   Some children behave like this to get attention.  We have to be careful that we do not let her get spoiled in this way.”

(As you start to examine the child, she screams louder)

“I am having a hard time concentrating while she is screaming, so would you please try to get her to stop crying.  I am trying to examine, and if she keeps yelling I cannot listen to her lungs or heart.  I need you to do a better job holding her down, or I will not be able to examine her fully.”
