In 2010, multiple branches of a new health care regime was implemented and designed to improve accessibility to healthcare for under-served populations including the 6 domains of quality healthcare established by the Institute of Medicine. There are multiple disparities and barriers that impede us from providing (and patients from receiving) quality care.  One of the greatest barriers in providing quality care is that of effective communication.Â
A quote by George Bernard Shaw states “The single biggest problem in communication is the illusion that it has taken place”. Â This is defiantly a true concept in health care. How often does a patient perform a urinalysis or blood work at an outpatient clinic, then repeated in the ER, and again requested once arriving on the unit floor. Is this a effective or efficient use of time or resources? This is the same when sending patients to a specialist who repeat expensive exams simply because they couldn’t find the results from the initial testing. Â
The Department of Health and Human Services (HHS) characterizes under-served, vulnerable, and special needs populations as communities that include members of minority populations or individuals who have experienced health disparities. The concept of Under-served Populations includes a broad array of populations who face barriers to health care services. Barriers to healthcare could be associated with ones geographic location, language barriers, educational development, age or sexual orientation, culture, race, or ethnicity, religion, fiscal or economic limitations, disabilities, alienage barriers, or those with special needs like technology or health literacy limitationsÂ
A. Identify populations and community groups that are vulnerable who are risk of being under-served or lack access to the resources in today’s healthcare system.Â
Â
B. Delineate those social determinants that act as barriers to for these population to obtain care in today’s healthcare system.Â
C. What can you as a nurse do to identify and address these health disparities to provide quality patient centered care for those vulnerable populations under your direct care?
