Oliver a 32 y/o male presents to psych ER with his husband. He is married, two childs 1y/o and 4y/o. He has an MBA (master bussiness administration) and a successfull carreer in comercial real estate.

Chief complaint: strees and mood swings

 

“I’m really stress, my mood swings are all over the place. One month ago, I ask my husband for a divorce due to an affair that I had. That lasted a couple of weeks but I could not lie to him. At the time everything was going fine, I launched a bussiness with a new partner. I was making a lot of investements. The affair just happened along with it. I felt like I had a lucky streak. I felt on top of the world.”

 

“A few months ago my PCP put me on escitalopram 20mg after diagnosing me with depression and anxiety. Within a week, I felt amazing. Before that my first episode of depression was when I was in college. At that time, It took me a long time to find a job. To cope with the stress, I was drinking and using substances pretty heavily.”

 

“I don’t know a lot about my family history. Im not close to my mom but she had a diangnosis of anxiety. My father left when I was 5 y/o and I know he was hospitalized for a psych disorder, for which I dont know the diagnosis that it was.”
