Case study 47 year old male, farm worker and single source of income for a family of 4; with history of hypertension,diabetes ,and gout; complained of sudden severe headache while at work accompanied with nausea and vomiting.patient quickly lost consciousness and was taken to nearby hospital,Kendall regional.A brain ct was performed which showed a massive subdural hemorrhage.patient was transferred to JMH for high-level of care.Brain ct/cta was obtained at JMH which revealed a large AVM abutting the cerebellum.patient was taken to the angiosuite where he underwent embolization followed by open section the following day.postoperative course was complicated by wound infection which required exploration and wash out.thereafter patient made progressive neurological recovery.he remained awake , alert and oriented times four.no facial deficits noted.Heart and lung function were within normal limits.Rehabilitation services were iniated to work on restoring endurance and stamina.Patient medications included pro tramadol for pain, amlopidine,lisinopril, metfoormin, glipizide,hydrochlorothiazide, allopurinol, copacetic and a multivatemin..please write up a nursing note to include the following: Chief complaint History of present illness Review of systems Physical examination Nursing diagnosis Medical diagnosis Nursing intervention Discharge planning Please also indicate various leadership qualities that you may utilize in caring for this patient.
