This is Scenario: 

A newly hired Registered Nurse is being given orientation and training in the 
hospital by a senior member of the Nursing staff. Assigned to a medical-
surgical unit, the new RN receives instructions on commonly practiced 
procedures in this unit. The New RN is instructed to “push Heparin IV” 
through a ‘saline lock’ in the patient’s forearm. 
The New RN, during her Nursing program, was taught not to do so such a 
thing as it could have serious consequences. When the newly hired RN 
questioned this procedure, she was told by the senior Nurse: “Look, this is 
how we do it here. We are understaffed and do not have the time to do it 
the way you were taught in school. If you want to work here, this is how you 
will do it too.”
