Upon every patient’s admission to the hospital, a complete health summary, including home medications, is recorded by the nurse and physician (O’Shea et al., 2021). The patient is often asked so many times about their home medications and health history facts that they become angry. Upon discharge from the hospital, the patient is given specific discharge instructions, including a new medication list that may have home medications to omit from their daily regimen (O’Shea et al., 2021). Without the EHR for medication reconciliation, preadmission home medications could be left off the discharge instruction by mistake (O’Shea et al., 2021). This medication error would be considered (O’Shea et al., 2021). The EHR is a safeguard within the discharge process to ensure all needed medications are listed on the discharge instructions (O’Shea et al., 2021).
Improves care coordination
The medical field is a fast pasted environment, and it is sometimes hard to keep everything about multiple patients straight. This is especially true at shift change while reporting to the oncoming staff. The EHR has been shown to prevent medical errors by showing real-time information about the patient in question during report (Wu, et al., 2019). When the current labs, medications, and diagnosis are on the screen for both providers to see, there is no issue with getting patients mixed up or numbers wrong (Wu, et al., 2019). Thus, reducing medical errors due to incorrect hand-off reports.

The EHR gives instant access to a patient’s medical record, including labs, medications, vital signs, and health history (Robichaux, et al., 2019). In cases of rapid response or a code, the patient’s vital history can be accessed at a moment’s notice compared to the old paper chart system (Robichaux, et al., 2019). Using the EHR, doctors and nurses can better document and provide a clear picture of the patient’s health status (Robichaux, et al., 2019).

Not user friendly
The utilization of the EHR has come with its own set of issues. Nurses in the hospital complained that the EHR had so many places to chart that it took them more time than just writing a narrative note (Robichaux, et al., 2019). The nurses stated that they found ways around the safety features to save time, defeating the purpose of the EHR (Robichaux, et al., 2019).
Training issues
Hospitals have a limited number of hours that providers can be pulled away from patient care to attend training sessions on new EHRs (Longhurst, et al., 2019). Not having the proper training leads to slower workflow, frustration, and workarounds to complete daily nursing documentation (Longhurst, et al., 2019). Each of these issues defeats the purpose of the EHR safety features. When providers are unhappy with the EHR, their job satisfaction can suffer, leading to poor morale (Longhurst, et al., 2019).
Risk of privacy issues
In recent years hospitals have been the target of hackers. There have been instances hospitals have been blackmailed with the threat of leaking patient information on the black market (Choi, et al., 2019). Efforts to secure data have included encrypted messaging systems, two identifier log-on, and having providers lock up portable devices at the end of shift (Choi, et al., 2019). These tasks seem simple, but in the hectic healthcare world, they may seem like another task added to a busy shift (Choi, et al., 2019). Unsecured devices have been linked to private information getting into the wrong hands and breaching confidence in the EHR system (Choi, et al., 2019).

Provide patients with electronic access to their health information and patient-specific education
In my APN clinical practice, I want to empower my patients to participate in their healthcare. Patients that look at their health trends may feel more inclined to change bad habits, take medications as prescribed, or lose weight. These are the main factors that lead to chronic health problems. I want to provide my patients with the best chance for a healthy existence. Providing patient-specific education to smokers or diabetics could encourage them to make better choices. Patients in the clinical setting are often nervous and do not hear all of what the provider is saying. Giving access to the same teaching material in the electronic portal could improve compliance.
Generate and transmit prescriptions electronically
In my APN practice, I will use electronically transmitted prescriptions. The electronic prescription helps to omit handwriting errors or lost pieces of paper. The days of prescription pads have ended with the opioid epidemic. I feel the new way of electronic prescriptions is a safeguard for our practice and our patients.
