John Derrick is a -y70ear-old retired Bank Manager. He underwent a transurethral
prostate resection 5 hours ago due to an enlarged prostate gland. He has a history
of Rheumatoid Arthritis and had rectal cancer and surgical treatment 3 years ago.
He underwent both chemo and radiation therapy at the time and has been cancer
free ever since. He currently has an IV running at 75 mls/hr and a 3-way catheter
in place with continuous bladder irrigation with small stringy clots. He complains
of a mild amount of pain in his groin and rates it 3/10 on the pain scale. His lungs
have decreased air entry to the bases bilaterally and you note on initial
assessment fine crackles to the right lower lobe. He has a strong cough and is on
02 2L via NP. His wife is at bedside and supporting him through his recovery. He is
denying any nausea and at present feels “comfortable”. He is on a clear liquid diet
but does not wish to eat at this time. VS are to be done Q4h and he has an US of
his bladder scheduled for tomorrow AM. At 1 PM you note no more drainage in
his urinary bag since your last check 2 hours ago. His bladder is palpable and he is
reporting mild pressure in his abdomen. The physician has left an order to hand
irrigate the catheter PRN to keep it patent.

Medications:
Ibuprofen 400 mg PO Q6h prn
Methotrexate 30 mg PO daily in AM
Ditropan 10 PO TID prn
Dimenhydrinate 25-50 mg IM/IV Q4h prn nausea
Hydromorphone 1-2 mg SC Q3 h prn pain
