You return to Ben’s room at 0830 and he is not in the room. You find Ben down the hall wandering aimlessly. You call his name and he does not answer. He is pale and has sweat across his forehead. You take him by the hand to walk him back to his room and his hands are trembling. You have him lay down in his bed and call the nursing assistant to bring in the glucometer to check his blood sugar. It is 50. His breakfast tray is sitting on his bedside table untouched. The midnight nurse had given the patient his insulin at 0645 when Ben called for his breakfast tray. Ben does not answer any of your questions but follows you with his eyes. He is able to open his mouth and swallow sips of his milk from the tray and then is able eat bites of toast and his cereal with you feeding him. He has no difficulty swallowing the food although done so in a slow-motion manner. He eats all of his cereal, all of the toast and a banana and drinks all of his milk and some sips of coffee. After about 15 minutes, he starts asking you what happened. He is alert and no longer sweating. There is no trembling noted of the hands as he is able to drink the rest of his coffee independently. A repeat check of his BS with the glucometer shows a reading of 108. His attending physician, Dr. Amil enters the room just after obtaining the second reading and is notified of what transpired.
