Based on how you will evaluate your EBP project, which independent and dependent variables do you need to collect? Why?

My EBP project is identifying barriers to medication, diet, and exercise adherence in patients 30 yr and older diagnosis with hypertension.  The project will last 6 months and includes addressing patients individually to identify barriers, and gathering information from individuals through provider-patient engagements, and EHR monitoring, the goal is to increase adherence, increase health literacy and disease management and reduce ER visits for hypertension crisis by 25%  by the end if the 6 month period.  Please cite sources used for  references
