Description of the purpose:
The purpose of this quantitative research study is to evaluate the usefulness of the COPD Web for adult chronic obstructive pulmonary patients. The COPD Web promotes self-management and knowledge about the disease. The aim of the Web was also to alter disease management strategies for patients. This specific study is small, but there are plans to utilize it for a future large-scale trial.
Explanation of research design:
The research design of this study is a parallel-group controlled pragmatic pilot trial.
Discussion of sample:
The study took place in the middle and northern parts of Sweden at six primary care centers. Intervention participants were all diagnosed with COPD and were patients who had visited the centers from January 15-May 15, 2016. The control group included patients who visited from August 1-December 30, 2015. Participants in the control group were asked to join the study by the study authors while the intervention group was asked to participate by health professionals.
Description of Data Collection Methods:
One of the data collection methods was the COPD Assessment Test (CAT). This tool was utilized because it has been shown to be responsive to healthcare interventions aimed to support management. The Swedish guidelines highly regard CAT as a crucial tool used for the management of COPD. Another data collection method included the Swedish Critical Health Literacy scale which was a standardized questionnaire specifically created for this study. Some of the other specific models utilized for the collection of data includes the linear response model, the ordinal logistic model, and the binary logistic model.
Summary of Findings:
It was found that 95% of COPD patients in the intervention group enrolled in the COPD Web. The area that was noticed to be affected the most in the intervention group compared to the control group was the physical activity portion. The Web was very useful in promoting physical activity to the intervention group, which made it effective. It was also noted that patients in the intervention group had a much better understanding of the COPD disease process compared to the control group. Approximately three out of the four patients with COPD in the intervention group utilized the Web for the initial three months.
Strength of the study:
A strength of this study is the fact that there were six Swedish primary care centers utilized and each one had a different geographical location and were each located in differently populated areas. The patients included in this study were not selected on specific exclusion criteria which increases the validity of the trial.
Limitation of the study:
The small size of this study can be considered a limitation. Sample size is very important to the results and validity of the research. Non-significant results of the study can be very misleading when the sample size is too small.
Recommendations regarding potential application for future practice that are insightful and appropriate:
This study supports the fact that many patients will utilize tools in order to aid them with a diagnosis. The COPD Web was created for COPD patients in order for them to have another resource to refer to when it comes to the management of their disease. Physical activity was seen to greatly increase in the intervention group that utilized the COPD Web, thus proving that this intervention was very helpful. An increase in the conceptual knowledge of the disease was also noted by the intervention group which is very positive to see. Education is extremely important when it comes to the care of a COPD patient and the COPD Web further proves this point.
