Scenario

Other members that wound be considered to provide Ertha better care is an OT to assist with ADL’s, a dietician to plan out meals for Ertha and establish a routine to implement nutrition, PT to provide a safe space and effective exercises for Ertha to remain active.
What specific issues would you want to take into consideration to provide for this patient’s unique care needs? Ertha is experiencing “disturbances in executive functioning, which is the ability to think abstractly and to plan, initiate sequence, monitor and stop complex behavior.” (Videbeck, p. 459) Safety is the primary issue to address. Ertha is striking out at other residents. She is in danger of hurting others or herself. Another issue that needs to be addressed is her medication regimen. Ertha has been on her medications for 4 months with no improvement. Her physician should be called to make changes to her medications, considering her symptoms.Â
How will you communicate with the patient? First, Ertha’s environment should be reduced of sensory overload. Ertha should be reassured that she is safe and should be reoriented about her situation and surroundings. It is optimal to use a calm voice and short, simple sentences to communicate with patients with dementia.
