A 40 year old man presents to your office with complaints of having 8-10 episodes of watery, non bloody diarrhea that started two days prior. He vomited twice yesterday and has not been able to eat all day except for some sips of water. He complains of abdominal cramps, rated a 5/10, that are intermittent. He feels weak, has a mild headache and feels like he has a fever.
He has no significant PMH, on no medications, does not smoke or and only drinks alcohol socially. He returned from a family vacation in Mexico the day before his symptoms started.
PE:
Alert, flushed, lips are dry
BP 110/60 AP 92 resp 16 temp 99.1
Lungs- LSC, no wheezing
Cardiac- RRR, no murmurs.
Abdomen- soft, BS hyperactive with mild tenderness to palpation. No rebound tenderness, no guarding.
Stool guiac negative
